GYN Appointment Forms
If you have not been to The Birth Center in one year, please call (302-658-2229) to make an appointment, then fill out a new registration form and submit it electronically. When you come to your appointment, please be sure to bring your insurance card.
OB Appointment Forms
For those who are interested in prenatal care as new patients: please complete our Prenatal Screening Form. Once submitted it goes to our nursing staff for review. If more information is needed to determine eligibility, or you are not a candidate for care at The Birth Center, you will receive a phone call from the nurse. Once you are cleared to come to The Birth Center you will be directed to call 302-658-2229 for an appointment and our office staff will email you the necessary forms to complete.
If you are a returning prenatal patient please call 302-658-2229 to make an appointment, then fill out the registration and health history forms, and submit them electronically. Read through the HIPAA Policy, and electronically sign the HIPAA form and submit. We will copy your insurance card when you come in for your appointment, so make sure you have your insurance card with you.

Lactation Appointment Forms
If you are a first-time patient please fill out the registration forms and lactation visit intake form, read through the HIPAA Policy, and sign the HIPAA signature page. Bring all completed documents to your appointment along with your insurance card.
If your baby has a different insurance please bring the card.
